Don't think is mono: If it was mono, your most prominent symptoms would be fatigue, sore throat, difficulty swallowing and very swollen neck glands. You did not mention any of those.
How do you know you have an enlarged spleen?
Now, any difficulty breathing is of concern, since it can go from mild to bad to worse any minute- allergy, asthma?

Under left ribs: The spleen is hard to feel  unless it is very enlarged. It is a very soft organ but in some conditions it becomes very swollen and tense. In that case you can feel the edge of it below the left lower rib cage.

Referred pain to left shoulder has been described with splenic rupture.  Suggest contacting your physician ASAP to plan next steps, especially with pre-existing splenomegaly.  If continues to worsen head to hospital.

Even if the abdomen is sunken in and fully relaxed, most people have enough fat and/or ab muscle to prevent it from being visible even when it is easily felt. A very large spleen (gaucher's, schistosomiasis, malaria) might be seen as an asymmetry in the upper left quadrant.
