Gynecology: Not from an exam. Only by you telling them this part of your history.  Your gynecologist will ask questions about your sexual history, surgical history, medications and any other relevant things that would go with being a sexually active female.

Possibly: If abortion is by D&amp;C, there may be some signs on exam. If by medication, less likely by exam. But depending on how far along you are now, your pregnancy hormone (HCG) may still be elevated enough to be detectable by urine pregnancy test or blood test. Regardless, you should feel comfortable sharing the fact that you had an abortion with your gynecologist - we are here for you =)

Yes: This would normally be prescribed by or under the supervision of a gynecologist.  Of course, not every gynecologist will be willing to do this because of moral, social, or legal concerns.

Depends: Assuming the legal aspects of getting an abortion are fine, your OB-GYN can either do it or recommend someone who will do this for you.  You might get this information from a phone call to the OB-GYN.  Also your local public health agency might have this information.

after the person: Who did the abortion tells u that everything is ok from the procedure you should have a post op check with the doc who did the procedure.

Continue MD care: The Abortion MD will know by ultrasound if you passed all of the fetal tissue or not on follow up exam. If you return to her office, she can then determine if a D &amp; C (cleaning) is needed or not.

Consider keeping apt: Physicians are human and may have opinions about certain sensitive topics such as abortion, circumcision, sex before marriage, etc. If you like your physician, hopefully you can get past this issue with him/her. It may be as simple as explaining that you felt uncomfortable with his reaction.

i had an abortion almost 3 years ago with surical process.now i repair my hymen.can a doctor tell that i was peragnent or had an abortion.

It depends: I don't have any real experience with this, but it would probably depend on what type of surgery was done (there are several forms). A doctor could probably tell that you have had surgery, and if you've been pregnant previously. 
You should hopefully be able to be honest with your doctor as he/she needs to know your real medical history.
