Single shots in shoulders, knees etc are linited and helpful generally.
Large dose, or repetitive long term steroids orally or injection can have issues.

Yes : Yes - one of the side effects to corticosteroid (i.e. Cortisone) injections is lipoatrophy.  Lipoatrophy is the medical term for loss of fat under the skin.  The steroid medicine can dissolve the fat under the skin and cause a depression, or scar.  It can also affect the cells that make pigment, so that the overlying skin can look lighter.  This side effect typically occurs several months after the injection and can progress, somewaht, over several months.  In many patients, the area can fill back in with fat cells, but this can take several years. It is purely a cosmetic blemish and may very well improve with time.

: It could be a coincidence rather than the side effect of intraarticular steroid injection but we can't rule it out completely. Before having the steroid injection I wonder if you have taken any medication for the joint pain which is likely the culprit. If you still take med for joint pain, stop it; if you do not, take antacid (Mylanta or Maalox) for a couple days.

It is possible: but post menopausal bleeding should always be evaluated by your physician to rule out other causes.  
Stress can cause fluctuations in hormones which may cause spotting, fibroids, cervical polyps, and  sudden wt loss can also cause post menopausal spotting.

I do not think so: Most likely what are suffering from  a neuroma between the toes, the symptoms you mention are typical to that. Check it out.
