No: The nasal bones would only be broken if this was a planned part of the surgical procedure; for example, in a trauma patient with multiple facial fractures, including a broken nose.  But, it's not required for the majority of facial surgeries.

Yes: Two types of breaks (fractures) - displaced in which the bones don't align and non-displaced where the bones are fractured but in good alignment.  In the acute setting, the surgeon takes a displaced fracture and reduces it (places it in proper alignment)  secondarily the patient and surgeon might choose to perfect your nose by cosmetic rhinoplasty.

Unfortunately, : Unfortunately, your nose is unlikely to return to its pre-injury position on its own. In the first 10 days or so it is possible for a surgeon to perform what is called a closed reduction. We don't typically do this bare handed, but rather use instruments to reposition the deviated nasal structure without making any incisions.

You may have a deviated septum as well, which would lead to nasal obstruction. Sometimes this will improve with resetting the nose, but a septoplasty may ultimately be needed to fully improve your breathing.

This : This is a common question among many people who are unable to breath through their nose. Commonly it needs to be established that there is a structural/anatomic reason for the airway obstruction versus a reactive/tissue response causation. Often patients will need to undergo medical therapy to optimize airway breathing as inflamed tissues can cause nasal obstruction as well. If there are clear structural obstructions then a submission to your insurance provider can be made to review coverage. Hopefully this is helpful to you.

Since : Since he has such persistent symptoms, I am glad that he is going back in to be seen.  He may need a scope procedure to look into the sinus areas and see what the problem is.  I assume he is taking a decongestant as well; if not he should be.  If nothing is seen by scoping him, he may indeed need another ct scan to evaluate his persistent symptoms.

It : It sounds like you have narrowing of the right nasal airway due to your prior injury. This isn't something you have to have fixed if it doesn't bother you. The main concern would be if a separate mass was seen growing in the nose. You could always get a second opinion from a different ENT doctor to see what your options are.

No : Surgery is not indicated for an uncomplicated nondisplaced nasal bone fx, at least not in the acute setting. If a deformity ensues & cosmetics is a concern, can repeat x-ray once inflammation has subsided or a ct scan for a more detailed evaluation. At that point can consult a facial plastic surgeon to discuss your options. For cosmetic purposes better to hold off on surgery until swelling is gone.

Anything if no conce: The reason to take precautions after a nose fracture is to prevent further deformity or compromise breathing. Plenty of people survive fine with a nose fracturelq without any problems other than a minor deformity. However i'f you are concerned than you should avoid direct trauma for at least 5 weeks.

History, exam, force: There are two reasons to fix a broken nose, if you don't like the way that it workand/or how it looks. I see many patients who have injured their noses and I do not believe x-rays have any role to play in the simple broken nose. If the nose is hit and there's bleeding and swelling then the nose was broken. If after the swelling goes down it looks okay and you can breathe forget about it. Otherwise surgery can put the bones back.
