what exercise can be done with the child  in full leg casts with botox  for 4 weeks?

Depends upon age: It depends upon your child's age, and his general health.  I am assuming that he is in the casts wtih Botox to manage cerebral palsy?  Does he have normal function in his upper body?  Talk with your local pt and they can help you devise some exercises that will fit within his ability.

Motion & quadstrengt: After immobilization in a cast you will
be stiff and have muscular atrophy.
The only good part about the cast is that
it helps the injured area to heal - it makes
the surrounding areas worse due to lack of
use.
You will need therapy to regain your motion
and to gradually increase your quad and 
hamstring strength to be able to regain
your normal activities.

Calf size: Your calf which has been in the cast is smaller because of muscle atrophy. If you don't use it, you lose it.  You just need to get on an exercise program to build up these muscles.

Cramps: Taking magnesium as a supplement reduces or prevents muscle cramping. Ask your physician or pharmacist to recommend a product which you can get without a prescription.

Need to be seen: You need to see your doctor / orthopod who placed the cast, an x-ray or change of cast may be needed, good luck

: I'm not surprised at the limitation of motion by the knee joint, but one worries about the presence of.phlebitis when calf pain swelling or tenderness follows a period of.reduced mobilization and prolonged cast wearing. Get checked by your MD who would probably order a leg ultrasound depending upon physical findings. Your knee joint will improve with appropriate physical therapy when approved byMD

is it possible to sprain or injure ankle (or any part of the protected leg) if the leg from calf to toes is in a cast (for 6 weeks and counting) and i fell on the side my cast is on?

: It possible to injure yourself even if the extremity is in a cast. If the cast is poorly fitted it could be possible. If concerns have cast removed and X-ray. Only way to be definitive

: I've never seen that with lower leg,ankle casts and I think you should be examined and advised by your physician or orthopedic specialist.

It is best: to not smoke at all. Would be a good time to quit.
