Possible cancer: A hyperechoic lesion should be evaluated. Check to see if Hgb increasing or blood in urine. An angiogram of kidney should be performed because of increased vascularity of kidney tumors and a PET scan would also indicate if lesion is possibly malignant. A needle bx then required to make histologic dx.

Yes.: The term renal indicates that the calculus is in the kidney. Mid refers to the midpole, or near the center of the kidney, as opposed to the upper or lower poles. If the stone were to pass into the ureter, it would be reported as a ureteral stone. A 6mm stone may pass on its own, but chances decrease above 5mm. If there are no symptoms it may not need treatment, but many options are available.

Not likely a stone: The lack of shadowing generally means that it is not a Stone. Is it a complex cyst or a tumor or a benign mass? That depends on a myriad of factors. While doctors may have their suspicions, the proof is in a biopsy. If the pt is otherwise healthy, it may not be worthwhile to biopsy an incidental lesion. TTYD or use HealthTap Prime. Good luck!

Renal colic?: If this pain to which you refer is on the right side in the mid-back, then I would think it realistic to presume this being due to your kidney stone.  The pain associated with a kidney stone, termed "renal colic" tends to start in the mid-back and shoot down and around to the front.  But, having pain would be contingent on that stone trying to pass, and not if it's just sitting up in the kidney.

Pyelonephritis?: Some UTI bacteria are resistant to nitrofurantoin (Microbid). It sounds like your infection might not have been adequately treated and now has ascended to involve the kidney (pyelonephritis). Return to your doctor ASAP.
