Mod prot in urine: There are two main components to kidney disease, which predict progression &amp; treatment. One is function or the stage 1-5 with one nl and 5 severe &amp; on or close to dialysis. Stage 2 in G2 is still considered mild &amp; low risk. We next look at a protein called albumin and 23 g/mmol is moderate (&gt;30) severe. In your question, you write CKD G2A3, I can't comment on the G, but 23 g/mmol is A2 - on tx?

Needs confirmation : It means you have around 2 gm of protien in the urine which is very abnormal. This needs a confirmation by repeating the test and be sure you don't have other reason to explain it like UTI or blood in the urine. If confirmed, then needs comprehensive work up to determine the reason. Diabetes is very common one but there are multiple systemic and/or kidney diseases that could cuase it

Depends on context.: Lots of things can cause slight to mild proteinuria. In the context of diabetes this can be a sign of significant kidney problems from the diabetes and an early indicator that the kidneys are being damaged. Or... some contamination from menses. Needs to be put into the entire perspective.

Significant: The protein/creatinine ratio is 4.6, which roughly corresponds to 4-5 grams of protein per 24 hours. This is clearly abnormal and consistent with the nephrotic syndrome.  This requires investigation by Nephrology, likely to include a biopsy of the kidney (a safe and common procedure).

Proteinuria: You have proteinuria (p) and the first thing is to have a 24 hour urine collection to see the amount of protein in your urine and to assess your kidney function. Have a renal ultrasound to assess kidneys' size
depending on the p, i would send off blood for aso, anca 2, complement C3 and c4, hiv, hepatitis b antibody to see if a cause could be found for the p, if not a kidney biopsy may be needed.

ONLY PROTEIN : Standard urine dipstick only measures protein by colorimetric method. If urine is postive for protein, exclude false positive, eg fever, uti, exercise postural. If positive, quantify it by measuring both protein and creatinine in the urine. .
 1+ corresponds to about 200-500 mg/24 hours, a 2+ to 0.5-1.5 gm/24 hours, a 3+ to 2-5 gm/24 hours, and a 4+ represents 7 gm/24 hours or greater.
