constant right sided burning rib/back pain. worse with activity. both sides of abdomen feel pinching. going on for two months now. cant sleep or eat much. lost 20 pounds. dr assumes ulcer. im on prilosec. what could it be?

Evaluation: If you have had persistent abdominal pain and this has been associated with twenty pound weight loss then you should be evaluated. 
You should probably see a gastroenterologist for an endoscopy.

Gastroenteritis: You may be having a viral illness, make sure keep yourself well hydrated, may try pepto bismol, (bismuth subsalicylate) Prilosec OTC. If need prescription meds need to all the consult line or see your primary care

i've just had a gastroscopy for coeliac. showed gastritis. bloated and severe stitch pain in side. sometimes pain / burning under ribs. please help!

Gastritis: Celiac disease is not diagnosed by gastroscopy since it involves the small intestines and not the upper GI tract. You problem appears to be from inflammation in the stomach which may be associated with  H pylori infection, NSAIDs intake etc.  It is best for you to consult your doctor or see a GI specialist to get a more focused diagnosis and treatment.

Hmmm: Could be kidney or gallstones. If the pain is severe urgent assessment at the Emergency Room is indicated. Otherwise if tolerable follow up with your doctor for further management.

Hard to tell: It is difficult to be specific diagnostically when the symptoms are so vague. This could be many different things including musical skeletal, splenic, GI or skin (Shingles). Need more history and perhaps examination to tell but probably not cardiac. Abdominal pain : Gastroparesis generally causes severe abdominal pain and persistent vomiting. It is often seen is patients with diabetes and patients using drugs. You may require investigations, such as a scope. There are medications and life style changes that can lead to improvement.

GERD: Sounds like gastro-esophageal reflux disease . Essentially the acidic gastric contents passing up into the distal esophagus due to an incompitent  esophageal  gastric junction. The distal esophagus has a different tissue lining than the stomach and can't tolerate the acidic reflux and this causes burning sensation.
