i have pain in top of big toe joint. feels like something small is floating around above joint. been over a year. heavy activity makes it worse. ideas?

Big Toe Pain: Good morning. If there is a history of trauma, their is a higher incidence of early arthritic conditions such as Hallux Limitus that cause bone spurs to form in the big toe joint, limit motion, cause "bumps" to occur over the joint, and cause more pain with activity. An x-ray would be taken to evaluate the joint and surrounding bony structures.

Cyst: One examination by an orthopaedist will give u your answer. Sounds like a ganglion cyst @ your age. Older patients in this location may be a mucous or degenerative ( arthritic) cyst. Other 'nodules' etc. may be in play with a more detailed history / examination. Get examined ! Best of Luck!

It's : It's always a challenge trying to give specific answers without the benefit of an examination or the ability to ask you specific questions.

That being said, the fact that you can run probably excludes any vascular issue like poor circulation (which is unlikely in a younger person).  Knowing you have flat feet, and given the widespread involvement that is limited to only your lower extremities, the first thing i would investigate is a biomechanical cause for your symptoms.  If your foot structure is such that it's causing your leg muscles to overwork in an effort to compensate for the structural issues, that most certainly can cause all sorts of muscle fatigue, heel pain, shin splints, low-grade tendonitis, etc.

I would recommend seeing a podiatrist and get a biomechanical examination.  Orthotics, either a good over-the-counter kind, or a custom molded pair will improve your symptoms dramatically.  Also, needless to say, make sure you take enough time to stretch before and after each run.

Get at the cause: See your doctor for an evaluation.  X-rays and/or lab tests may be necessary to diagnose what exactly is going on.  It sounds like arthritis, and there are many different types of arthritis.  If it is at the base of the joint, and there is bump on the side, it could be a bunion.  See your doctor for a more specific answer.

Technically : The big toe joint would be the interphalangeal joint not the mpj, but, I'm not sure which you are referring to. If the ip joint, I'd look to see if the toe was straight or not. If not internal compressive forces may be irritating the joint. If mpj is bothering you look for dorsal spurring to be a cause, I.e hallux limitus. Gout also affects this joint.

Depends: Any surgery especially to the big toe has its advantages and disadvantages.,,with the implant ur doctor should discuss longevity of an implant and loss of toe purchase power... I prefer Cheilectomy or fusion...implants have there place but it depends on patient expectations ...

Fungal infection: Perhaps this is a fungal infection of the toes.  Is the itching worse after wearing socks & shoes for several hours?  Is it worse at the end of the day (assuming you work daytime hours)? Any redness, irritation, peeling skin?  It doesn't have to have a foul odor.  If yes to most of these questions, consider a fungal infection. Try some otc remedies first.  If no improvement, see a podiatrist.
