Skin discoloration: should be examined by your doctor.  If it is changing in color, texture, temperature and it itches I recommend getting seen as soon as you can.  You will likely need a skin biopsy to give you a correct diagnosis and treatment plan!

Poss folliculitis: This may be folliculitis (inflammation/infection of the hair follicles.).  It can resolve by itself, and warm compresses/topical antibiotics (like Bacitracin) may be helpful. If severe, it may need to be seen by a physician.

theres a red rash on my right arm, its hot to the touch. it looks like sunburn & radiates heat. its gotten larger but comes & goes. my right ear & shoulder get the same warm red rash. theres no bumps, itching, or pain but should i see my pcp?

Skin infection: This could be a bacterial (strep or staph) infection of the skin, and can be very dangerous. If there is any evidence of a red streak going up and down the affected limb, see your MD immediately.

Hard to know: Thigh mass 
swelling
could be a blod clot
could be an infection
could be a mass
see a doctor if the mass persists.

In the case of a clot since this can go up to your lung you might want to see the doctor sooner.

This is likely: Do get this seen by your physician at your yearly physical. We want to be sure it's not pre-cancer -- this pops up as little red splotches on the glans. I admire and respect you as an outdoor worker -- thanks for what you do. Some talcum / gold bond powder to the area before and after can work minor miracles for heat rashes on a guy.

Many possibilities: There are many things that could cause the skin to itch.  Several types of inflammatory conditions of the skin, allergic reactions, and sometimes bacterial, viral or fungal infections can cause rashes on the skin. Insect bites can also be the cause of a rash on the skin.  Have it evaluated by a professional and get the appropriate treatment.
