Wait for insurance: Don't pay before insurance completes their transaction.  By contract, the physician or hospital must accept the insurance maximum for that service. This means the overall charge may be reduced.  Wait for the insurance to pay the doctor first or call the insurance company to verify your obligation. After insurance pays, you will get a revised bill.  If you are due a refund, demand it - it's the law.

Insurances: Sometimes that may be the case. It is unfortunate how insurances and manufacturers manage health issues with a market mentality; their focus is not always health care, but plain business. However that is how the system is structured, and every dr wish every patient could afford all necessary medication. Cost is a factor, we often need to adjust, use generic or alternatives our patients can afford.

Send to collection: You probably signed a document on your 1st visit with your doctor that you agree to be financially responsible for your medical care. It's like all the agreements and contracts we constantly sign with all the fine print. I doubt you planned to get your medical care for free. Communicate with the office and arrangements can be made. By the way, it is sometimes illegal not to charge you for  care.

I : I think it would be preferable to go to a free clinic in atlanta to have this handled by a medical professional over treating yourself with an on-line product.  Think about it, that's your genitalia. You don't have medical training. I do think it is smart to get this treated however.

Bill: The price seems high.  A comprehensive exam at our office is $225 for a cash pay patient.  They may have performed additional tests which could add to the bill.

Doctor: A doctor did read your ultrasound, even if you didn't meet that person.  An ultrasonographer usually takes the images, and a doctor may or may not be in the room for that part, but if they are not there, they review the images later and decide whether the exam is normal or not, and make recommendations for follow-up if necessary.

Gradual: Gradual increase your activities doing things that you enjoy and are fun for you.  Also consider using a trainer or therapist if you feel uncomfortable doing it alone.

Free clinic: Call your local health department and see if they can help you find a free clinic or community health center. They may be able to see you at little or no cost and help you get treatment for whatever condition you have.
