Occipital Neuralgia: Sounds like you might have this.  I would consider icing the area and using anti-inflammatories to help.  If it does not resolve you might see your doctor for a occipital nerve block with steroid.

Maybe infection?: Or spontaneous bleeding?  Tough to say without looking at it.  Sometimes shingles can affect nerves in the area.  If it involves eyes, it can be serious.

Try...: If it is smallpox no more than 1 cm) you may try to use warm compresses for a few minutes there times a day and over the counter antibiotic ointment( for example Neosporin). If there is a lot of pus, redness, a large bump and pain then you will need to see your doctor  or go to the urgent care for possible antibiotic treatment.

A bit more than that: A serious blow to the temple that results in a skull fracture can lacerate the middle meningeal artery and cause an epidural hematoma. This is a neurosurgical emergency that must be evaluated and treated promptly or a person can slip into a coma and die. A slower subdural hematoma can cause issues as well. Any head trauma with changes in mental status should be evaluated promptly.

It could be caused by low platelets, serious infections, other blood problems. If he is acting ill at all, you need to seek medical care tonight. Some of these illnesses can cause serious disease within just a few hours. So if ill, fever, malaise... Get care right away.
