This is due to the Insulin resistance syndrome, get checked for this and follow a low-glycemic index diet and get frequent, aerobic exercise. But why so much pain with defecation? You need to see a gastroenterologist to figure this part of it out.

Fissure, maybe...: You may have a fissure- an acute crack or tear in the anal canal that is quite painful. Spasm of pelvic floor muscles and nonrelaxation of the sphincter can make it difficult to move bowels. Drink plenty of water, try a stool sotener, but above all, see your doctor asap for diagnosis in person, and to rule out more concerning issues that may be present.

See a doctor: your rectal pain could be anal fissure,or a hemorrhoid.
your upper abdominal pain could be an ulcer,or your gallbladder.
eat light food,avoid spicy and greasy food,drink a lot of water,and juices,eat fruits and vegetables,eat more fibers,keep your bowel soft,
check with your doctor for some testing

If persists&gt; see Dr: There are a number of conditions that can cause rectal pain. If the pain occurred only once and you don't have pain on defecating it's possible the pain may be due to Proctalgia fugax. This can be due to spasm of the anal muscles, nerve compression or other causes. Rectal Pain: Many reasons could cause rectal pain such as hemorrhoids or anal fissures. You should see a gastroenterologist for a formal exam and possible anoscopy (endoscopic) evaluation

Proctalgia : Is many times generated through muscle spasm or nerve compression and work up is needed in order to understand if structural changes are present. Talk to your doc about your symptoms so that workup can be started.

what exactly is constipation? is it reduced bowel movements or none at all? that being said, can constipation cause rectal pain and pressure?

: Both could happen, decreased bowel movements, difficulty with bowel movements (even if you feel the urge).  If you don't have one at all for some time.... that's a little more serious and suggests a bowel obstruction.  Do remember "normal" bowel movements can have a significant range and still be considered "normal".  Usually people can vary between 3 times per day, down to 3 times per week.

R/o Colitis : Need to try to get you squeezed into a faster Gastroenterology appointment please call your insurance. Any vomiting of blood or dark tarry stools? Any lightheadedness or fatigue at rest or exertion. As a woman you will also lose blood via menstruation so see MD ASAP to make sure you’re not anemic ER if worsening fatigue shortness of breath or fever/chills if bad don’t hesitate to call 911

Constipation: If you are having severe constipation you want to make sure you are having enough fiber in your diet. You can add Benefiber or Miralax (polyethylene glycol) to help soften your stool.  Have your thyroid checked to make sure your constipation isn't due to any issues.  Have your back checked if you get the pain without the constipation. Feel better soon
