Female organ cancer: Gynecologist oncologist treats female related cancers in genetalia, female pelvic area like vagina, uterus, ovaries etc.  She/ he performs surgery and often gives chemotherapy as well. Breast is usually not treated by gyn/ onc.

No: Firstly there are multiple types of leukemia (acute, chronic, granulocytic, lymphocytic, hairy cell). There's a general consensus based on respected multi-insitutional centers on the latest successful regimens. But these are only guidelines. Each person is unique and rx may need to individualized. Also new studies r always being done, and study portocols will vary from one institution to another.

Depends: I'll defer to a medical oncologist regarding cost of chemo.  Cost of radiation can very from about $3000 for a 2 week course for simple bone metastesis to about $30-40, 000 for an 8 week course of imrt for prostate cancer.

Practitioner: Nurse practitioner can see you and can prescribe treatment while oncology nurse usually deliver chemotherapy or radiation but in actual she can not prescribe tratment by herself.

But many cancer centers run neuro-oncology programs. That is where you will find more specialists. So head to a major cancer center like md anderson or sloan kettering, mayo clinic, johns hopkins or, you can go to ucla or ucsf medical centers in california.
