Infected/inflammed: Red, hot, swollen and drainage (inflammation) are normal signs of healing and in fact, not bad but are integral to healing.  If infected, the wound should not improve, the patient have fevers, chills, and other systemic signs of infection.  Culture of the wound should never be with a swab on the surface but a simple tissue biopsy.  Routine use of antibiotics is to be condemned due to the risks.

Piercing scar: First remove the jewelry and allow the hole to shrink.  If it is still unacceptable then see a doctor about your options.  Board-certified plastic surgeons frequently treat these conditions.

Sure: You might be worried about an infection of a repaired coarct. I would speak with your heart doctor or surgeon before proceeding, because you may need antibiotics if there was another heart issue fixed at the same time (coarct is often linked to other problems). In most cases, you should be fine, though. A quick call is important before you do anything invasive.

Yes, It Depends.: Once a coarctation is repaired, surgically or by balloon dilation or stent placement the inside of the vessels grow and cover any foreign material (called endothelialization)  

once this has occured getting a tattoo or body piercing should be safe.  

However, an unrepaired coarctation with stenosis and turbulent flow has some risk for being a site where bacteria can land.
