Fissure o Hemorrhoid: Most common cause in a 21-year-old would be anal fissures or hemorrhoids.  Hemorrhoids can become thrombosed which requires a more urgent medical attention. Please seek medical care if your pain persists or worsening. Otherwise can try stool softers, otc products (wich hazel), and minimize straining. See if this helps until you see your doctor.

Get a full scope.: Most early colon cancers are asymptomatic.  The blood on the outside of your bowel movement is likely due to hemorrhoids. Colon cancer is possible, however, and the sigmoidoscopy you had only looked at half of your colon.  So to rule out colon cancer, i would recommend a full colonoscopy.

Stop "worrying": Stop "worrying" and see a MD for an exam. Rectal bleeding could have many possibilities, but an exam will be definitive.

: A tiny bit of very fresh looking blood on top of the stool or on the paper, rather than mixed in the stool, is generally a benign issue like a tiny anal fissure from hard stool or could be from a hemorrhoid. If it continues or if there are associated bowel symptoms, see your physician or a gastroenterologist for an examination.

: Some basics to understand. Bright red/pink blood means it is fresh &amp; from near the outlet. When it comes from up inside it has been acted on by inside chemicals/enzymes and changes through a darker maroon to purple to black, depending on how high up it originated. These are the facts &amp; they support what your 3 docs said, but did not explain. I would relax. This is common.

Most conditions with bright red blood, usually on the surface, are benign but uncomfortable and sometimes scary such as fissures, and hemorrhoids.  Blood can occur when there is lower bowel inflammation and rarely tumors.  If there is consistent blood in or on your poop, you should see your family doctor for analysis.
