Nail procedure: If the toenail is painful when you apply pressure then you need to have the pressure relieved to release the blood underneath the nail plate. It's possible to also have a toe fracture following such a traumatic event. See a podiatrist for a x-ray and possible nail procedure.

Under the toe: Or under the nail? If under a nail don't attempt yourself, see a diarist. If on the skin, i would leave it alone. You can soak it in warm water and day by day it should get smaller. This is not a true "blood clot".

Not likely: The symptoms you are describing are more likely to be tendonitis.

Get it checked: I'm not sure what you mean by raised. Is it under the toe nail or next to the toe nail.? It could be due to trauma or maybe a mole or even possibly a melanoma. Please have it checked by a dermatologist or plastic surgeon as it may require a biopsy.

Decreased blood flow: A person with on and off cold fingers and/or toes usually has occasional decreased blood flow to his hands/feet. This is not from blood clots, but is due to the temporary contraction of little arteries carrying blood to the hands/feet. The arteries relax and widen later, and the hands/feet warm up again. A primary care doctor or rheumatologist can evaluate, as it could be an autoimmune disorder.
