Need evaluation: Do not be too concerned, because small amounts of spotting from the surface of the cervix is common with intercourse.  That being said, i think that you should see an ob/gyn and have an exam and ultrasound to look for cervical disease and possible uterine polyps.

NO: It can take up to six weeks for the cervix to heal after a LEEP.  Be sure you followed up with your doctor postoperatively and that you abstain until they say your cervix is healed.

Not normal: Vaginal tears are not a "normal" event during intercourse.   Most likely you tore in this location and haven't ever completely healed.  This area seems to be prone to stretching and is a hard location to completely heal.   I've seen these persist for months.  You need to get an exam to make sure there is no infection.  Vaginal rest and topical estrogen also sometimes help.  Best wishes!

Get urine retested: It is important to check on the results of the initial urine culture used to establish the diagnosis of UTI. This will tell you if the antibiotic that you were prescribed was effective for the infection.  Since you are still having symptoms, seeing your doctor to get your urine rechecked for signs of continued infection is important.  Also would check for Mycoplasma/Ureaplasma infection.
