i have been bleeding heavily after sex. i got a pap smear and full exam and everything came back clean as far as infections, hpv, etc. what else could cause this? sex is not painful when this happens either. it is also fresh, bright red blood?

: Bleeding is a sign of tissue injury. The vaginal wall of a post adolescent female is estrogenized/toughened enough to handle intercourse. If you are bleeding, there is some form of tissue tearing, so you should have an evaluation to see where the blood is coming from.

It is uncommon: You may have bleeding after pap smear and then having sex but to have heavy bleeding , it is very uncommon . If it does not stop , i encourage you to see your physician.

Delayed menses.: Please get a pregnancy test .If you are pregnant the bleeding may be due to implantation  or threatened abortion. Most episodes of implantation bleeding stop spontaneously if you avoid sexual further intercourse. Threatened abortions usually also stop, especially if there are no pelvic cramps. If you are not pregnant you  may be having a menstrual cycle in which you did not ovulate.

Many possibilities: Many things can cause abnormal bleeding but after 2 months it is definitely time to be seen and look into it. This could be any thing from an infection, to thyroid disease, diabetes, PCOS, or even cervical or endometrial cancer. Get in to your PCP, gynecologist, of an urgent care clinic ASAP and begin the work-up. Good luck.

Cervical or uterine.: Cervical or uterine problem such as an infection.  See your doctor asap.

Can do that: implantation bleeding usually occurs 6-12 days after ovulation, is easily confused with normal menstruation, and can be complicated by vigorous sexual intercourse, depending on how vigorous the sex is. It's all in the timing !!

Talk to GYN!: Clearly you need to ask GYN what the plan is. Have you had an endometrial biopsy yet? Have you had your uterus examined with US for possible fibroids or adenomyosis?
