No: On the scalp it is common to have adherent scabs because of hairs trapped in the scab. It will be black from old blood. If you cannot tease it off or wash it off, your Dr can easily remove it. This may involve gently cutting hairs under the scab.

Melanoma: Was the mole itchy and that is why you scratched it? If so it may be worth getting the mole examined as itch is a change we look for in moles

Need details: Warts? Keratosis? Tiny blood scabs due to minor self-injury? Instead of picking you might begin to rub in a moisturizer cream. Carry a small tube and rub in 10 times a day, using a small amount and rubbing in hard. Are they moles or freckles or something else? Where? How many, how large?

Mole: A change in a mole, in this case , a scab forming may represent a cancerous mole or simply a normal mole that has become irritated.  In any case, see a dermatologist for evaluation.

May not be a mole: Mole is part of the skin , you can't scratch it off , is like a tattoo , see your doctor to find out what is  and to treat.

It depends: Moles can be cancerous if they are black/brown in color and changing over a period of time. If it has increased in size, then it is best to show it to a doctor who may do a biopsy(to confirm if it is melanoma) or may refer you to a Dermatologist who are better at making a reliable diagnosis and offer appropriate treatment which is usually wide local excision.
