See doctor: It could be a tick (in US) or other blood sucking insect, a bruise, or a cyst.  Someone should examine it.  Not likely to cause a headache, but could be.

Get seen: This is probably a dermatofibroma. They often pigment as the macrophages of which they are composed tend to take up iron, and the epidermis overlying them tends to accumulate melanin. If you want to be rid of it, your personal physician can remove it in the office.

No guessing..: Guessing what this might be is a disservice to you. It is not a normal part of your anatomy, and may be absolutely benign, or more serious, especially if you are a smoker or used chewing tobacco, which we do not know. Call an oral surgeon for an appointment asap.

Discoloration: A new discolored area in the vulva needs to be checked out. You need to see the Gyn for possible biopsy but at least for a doctor to view the area

Gyn exam: without a vaginal exam, I suggest a GYN evaluation: this may be a cyst, gowth or infection; however, must be examined.

Abnormal: It is difficult to guess what this could be without a visual examination, but this sounds abnormal enough that you should see your doctor or a gynecologist to see if this needs imaging or a biopsy. You'll want to know what potential this has to be a cancer. Hopefully you don't have any family history

No: You should follow-up with your doctor asap to work up the bleeding (and the black lump may be old blood), but the er is not likely to help you as there is no emergency.
