Yes: That would be higher than the recommended maximum dose of 3750mg a day.  Would expext higher incidence of side effects.  Whether a higher dose is more effective, I cannot say.

Time: If you have truly avoided all gluten, the small intestine will eventually heal up and start absorbing the nutrients within a few months at the latest.  Just give it time. If you still fail to gain weight after 3 months, check with your GI again to see if there may be something else causing your weight loss issue.

my b12 is at 200 ng/l lab reference is 130-800. i have bile acid malabsorption. is this level acceptable for everyone, or could i benefit from more ?

Could use more: It would be prudent to consult your doctor for the need to take supplemental vitamin B12. It can given as an injection, a pill to be swallowed or a pill kept under the tongue. Your doctor who knows more about your health would be able to advise better.

Ok, but...: There are no interactions between the two, so they can be used at the same time. But there is potential for side effects, especially of the metoclopramide. Therefore, you should follow whatever regimen your G.I. Dr. believes is best. Good luck.

: Go for an exam and testing to your PCP. Other diseases can cause similar symptoms. Stop worrying and seek professional help.

: Most people have low vitamin d because they work indoors and don’t supplement, so that is not proof of malabsorption. You need to consult with a doctor on the ground who can perform an in-depth work up for your many symptoms. Stool studies can be performed to rule out malabsorption.

Go see your doctor: If you keep vomiting bile, even after drinking just water, it's time to go see your doctor and undergo endoscopy (egd) to look for possible obstruction causing bilious emesis (vomit).

: Too much acid in your  colon can result in diarrhea and watery stool. If this is your case, see a gastroenterologist for exam and treatment.
