Chronic changes: The description is consistent with chronic changes, probably secondary to infection. With time the findings may improve. If you have sinus symptoms, consider an ENT appointment and nasal washing culture.

Thats fluid in the: Mastoid bone.  It is usuallty can be from infection in your ear.  You shoudl go see an ear doctor specialist.  Sometimes it is a bland fluid collection and not from infection but i would let a doctor decide that.  Also check if you have a fever at night.

Infection vs inflama: This could be infection vs inflammation You may have sinus infection. You need to see ENT. You may need a biopsy.

: If the ENT specialist you consulted is puzzled by these symptoms and signs, I recommend consulting a neuro otolaryngologist at the department of otolaryngology at a university hospital near you. Bring the MRI disc with you.

Pleural Effusion: It is collection of fluid in the in the pleura (lining around the lungs). 
Multiple causes: infection, tumor, systemic disease, heart failure, ... etc.
Depending on your age and your past medical history, investigations are carried out to find the cause of it.
If no cause is found, it is usually followed up for progression.

Radiology: These findings can only be utilized in the context of your clinical presentation. You may or may not need any treatment, but this should be determined by a doctor practicing clinical medicine, preferably ENT, and not by a radiologist looking at films without seeing and evaluating you. Arthritis: Joints in the spine are similar to joints in the rest of the body.  When they degenerate due to arthritis they become fluid filled.  That fluid is referred to an effusion.  An effusion is basically indicative of degeneration of that joint.  It can lead to pain, and in severe cases, instability of that vertebral level.

bilateral pneumonia complicated by pleural effusion-- can this be fatal?

Probably not...: This condition is typically treated with antibiotcs and drainage of the effusion with a chest tube if it is large enough. Generally it is not fatal; however, if severe enough & not responsive to treatment, this can be very serious.

every few months i get bilateral knee effusion that needs draining and a shot. the fluid noninflamatory. why would this keep recurring? no injury

Unexplained effusion: Have you had an MRI yet?  You may have articulat cartilage or a meniscal injury. Blood tests can help rule out rheumatologic diseases. Infection sounds unlikely. I recommend you to be evaluated by an orthopaedic doc or if you already have done this, get a 2nd opinion.
