It's the amount : There is no ideal form, or one specific vitamin. Vitamins like to work with others, so taking more of 'just one' isn't as good as taking a mutilple vitamin.  
If you are talking about healing after a surgery or burn, then for awhile higher than rda amounts may be useful - the body needs more - and more protein - during repair time.

Medical evaluation: A bulging disc, which in your case must have been medically evaluated to get this diagnosis, can cause local pain and muscle dysfunction or pain in the area of the nerves from this area.  Your doctor will recommend medication, physical therapy, cold applications and other measures for relief.  Vitamin d will make no difference.  Continue under your doctors direction.

Gastroenterology: Malnutrition due to "digestive issues" must first be defined and then treated according to the exact diagnosis. The rest of it is footnotes which will naturally be addressed as part of the central digestive issue.

husband recently began taking vitamin a 1 week ago to aid with wound healing per his md. he now has swollen hands with a rash that looks like hives. i gave benadryl. he told me he's been taking 2400 qid. doc said take 10,000 mcg. is this too much?

: Just for supplement, not treating Vitamin A deficiency, dose of 10,000 mcg a day is too high. The maximally recommended supplement dose is 3,000 mcg or 10,000 IU, daily.  
If rash is suspected due to the current vitamin A supplement, you may need to change to a different brand. High dose of vitamin A is not the reason for the rash.

C,folate, b complex: These may help repigmentation but are not required. Response is variable and the vitamins rarely work as solo treatment. Topical medications, narrow band uvb, excimer laser can be combined with or without vitamins to repigmentation localized vitiligo.

Unresolved question: Some people believe that the antioxidant effect of vitamin C and e may reduce the effects of radiation therapy.  This is not a proven effect.
