Not a bad idea: Always a good idea to establish with a doc, either GP or gynecologist if trying to conceive.  A thorough review of preexistent medical conditions, current meds and supplements as well as needed supplements once pregnant will only help in the process.

Ovulation prediction: The best way to know the exact day of ovulation is to get an ovulation prediction test at the pharmacy.  They are sold without a prescription.  Usually you start testing 12 days after the start of a period.  When it turns positive, you know ovulation will occur in the next 12-48 hours.  That night and the next are ideal times to "try".   Testing sperm is done in a lab.  See your doc for that!

Rhythm method: Sex on days close to the ovulation date is more favorable to become pregnant. This is the time which is mid cycle (day 15 or approximately 10 days after the period stops). Start  two days before and continue daily for 7- 8 days. The period one week before and one week after the period is least fertile and unlikely to result in a pregnancy...so you can skip this time.

Need more info: When women have miscarriages, they often have d&cs afterward.  Cauterization implies heat... Like an endometrial ablation (something your wife is too young for).  An enlarged uterus without fibroids is typically adenomyosis (similar to endometriosis).  The best treatment for adenomyosis is hormone management with birth control pills, or shots.  Definitely speak to a doctor if you or she is worried.

Keep trying: If she started the doxycycline right before or during her period and if you and your wife happen to get pregnant before you completely finish the doxycyline there should't be any problem since it will be too early in the pregnancy for the fetus to be exposed.  If she is close to ovulation then I would wait on trying until it is completed and after her next period.

Near or at the: Time of ovulation. This is 14 days prior to her period. She can use an ovulation prediction calendar off the internet or buy an ovulation predictor kit at the drug store.

Essure: The safest, most effective method with the lowest complication rate is called essure. This is done in the office and requires almost no down time. Most patients do not need pain medication after and can resume normal activity. In some cases we still go through the belly button under general anesthesia to clip or cut the tubes.

Fertility work up: Consider seeing your gyn about causes of her irregular periods.  You can also consider using ovulation prediction kits and start using around 13-14 days after her period begins and use for 5-7 days to see if she is ovulating.  Recording her cycles on a calender may help and consider some preconceptual prenatal vitamins.

Painful Intercourse: I appreciate you asking this question. No one wants their partner to have a painful experience with sexual intercourse.

She very easily could be entering menopause. Her vaginal tissue may be drier than you are used to and she may make less lubricating fluid in her vaginal glands.

This can assessed by her gynecologist.  She may be helped if you use lube while making love with her.
