Ibuprofen: Ibuprofen 600-800mg taken every six hours works great. If allergic to aspirin, nsaids or are taking blood thinners ask your doctor first. Acetaminophen (paracetamol) 650mg every 6 hours will help, but will not control the inflammation that causes the pain. If needed, you can take both if you don't exceed 3200mg Ibuprofen or 3000mg Acetaminophen per day. Talk to your dentist for specific help.

Try naproxen sodium: I recommend Naproxen sodium , aleve, (naproxen) as it will reduce both the pain and the inflammation  following the extraction. Plus you can get relief with fewer pills than ibuprofen.

Oral Surgeon: Get to the Oral Surgeon to properly diagnose the problem and initiate proper treatment. If you have an oral-antral fistula then it is paramount that you get there ASAP. These are serious and require expert treatment and management to resolve. Good Luck . Don't delay.

Not radiograph: It would be very unlikely to be able to find it with a regular radiograph.  You might see it in the ct scan.  Are you waiting for the area to heal before placing an implant?  Was the area bone grafted at the time of extraction?  If so, there probably won't be any sign of it.
