Loaded Question!: First, you should never get your tongue pierced.  There are so many chances for infection, disease transmission and dental issues resulting from that poor choice.  I realize many think it's "cool", but many times "cool costs lots-o-money"!  to answer your question specifically, yes; as long as there is no infection, swelling, or residual issues from the actual piercing.  Please take it out!

See an ENT specialis: You may need surgery to prevent a permanent slit. Maybe not. An exam by a throat DR will determine the best course. If surgery is needed, the sooner the better.

YES!: It is very painful during and afterwards. There is a high risk of infection as well. Expect pain, swelling, difficulty talking and eating .  If you place a metal stud, expect to break teeth as well. It is not a recommended procedure from this dentists point of view.

If the swelling makes it difficult to breath then you need emergent medical evaluation to ensure that your airway does not become compromised. If the tongue is seeping drainage that is other then white-colored or that is malodorous – then please seek medical care for a possible infection.

Nerve endings: There are a lot of nerve endings in the tongue, so it will hurt.  If this is a new pain in an old piercing, I would worry about infection, which could get very severe very fast.  Take out the piercing and see a doctor soon.

Pain now or later?: I would not do it.  I have seen many patients lose teeth, crack teeth, or loosen teeth later on.  

Of course, there could be an infection when the tongue is pierced, and then there would be bleeding, swelling and possibly loss of function.

Probably not: For the long term, probably not, but it will be temporarily changed.  Be careful with tongue piercings, as these increase your risk for cracking or fracturing your teeth.

Anti-inflammatory : Ibiprophen meds are fairly efficient in controlling pain and most importantly inflammation which is the bigger risk of tongue piercing. Be sure to follow up with an oral surgeon if you have any complications.
