Multiple options: The best idea is to stop migraines from developing.  Preventive treatments include: medications, procedures-botox, nutritional supplements, increased exercise, and avoidance of migraine triggers.  Treatments once a migraine develops include nsaid's (motrin), triptans (sumatriptan) amount others.  There is not, to my knowledge, a specific treatment for basilar migraine.  Get help-neurologist.

Depends: This depends on the specifics. Acutely, the issues are to support the patient  and prevent complications. ( make sure the patient is able to breath ok , that blood pressure is ok , etc) in some patients who get care withing 3-4.5 hrs of symptoms a clot busting drug may improve outcome. Long term rx involves rehab, risk factor control & use of aspirin, or even more potent drug to prevent blood clots.
