Mealtime insulin: A few suggestions:
1. Make sure diet is perfect and consistent. See dietician if needed
2. Work with your md to titrate up your basal insulin
3. You can take a short acting Insulin such as NovoLog or humulog at the time of meals to keep the sugar from skyrocketing after meals- this can sometimes be the cause of a stubborn a1c.
4.  Take metformin too if medically safe and not already on it.

i just injected 40 units of lantus (insulin glargine). it bled a decent drop until i pressed the area with my boxers. how can you tell if you inject into a vein?

Don't Worry: This is an old concern &amp; particularly with rapid insulin. With insulin pens &amp; small needles it is hard to hit a large vein.  You most likely hit a venule, a very small branch of a vein, &amp; the needle will actually pass through it. The insulin will deposit behind the vein the Lantus (insulin glargine) slowly release, but you will bleed a little. If Lantus (insulin glargine) is in a venule it will likely clot &amp; the Lantus (insulin glargine) work normally.

Probably not: 4mm of tonsilar herniation is not technically a chiari malformation as the cerebellar tonsils need to protrude more than 5mm to be abnormal.  The other findings sound like normal variants found on an mri.  If you are having symptoms you should see your doctor.

Diabetes: I prefer this term to diabetic.
Yes you do have diabetes and it is not yet fully controlled.

I would ask your doctor to refer you to a diabetes educator  and, if you have type 2 diabetes, work hard to get rid of diabetes.

Hypoglycemia: To diagnose insulinoma, you need to demonstrate inappropriately high insulin/C-peptide when your glucose is low. A glu of 81 is not low, it is normal. You have symptoms of hypo, but these symptoms are often from something else. Insulinoma is rare. Do not start getting CT/ultrasound etc until you establish the biochemical diagnosis first. An Endocrinologist can help you.

Doctorspeak: This is doctorspeak for normal findings on the MRI.
A flow void indicates there is blood flow. Lack of a flow void would be bad. This is normal.
Prominent perivascular spaces are likewise a benign (nothing to worry about) finding.

Humulin: Humulin is the marketed name by Eli Lilly and Company. Both are biosynthetic human insulin.
Humulin N, is neutral protamine hagedorn (NPH), an intermediate-acting insulin, has an onset of 1-3 hours, peak of 8 hours and duration of 12 to 16 hours. 
Humulin R is Regular insulin, a short-acting insulin, has an onset of 30 to 60 minutes, a peak of 2-4 hours, and a duration of 5-8 hours.

Different timing: R is usually given about 1/2 hr prior to a meal. No longer the short acting Insulin of choice because others more effective having shorter 1/2 lives. Nph may be combined with short acting Insulin or given alone to provide later coverage. Starts to peak in ~ 4 hrs, though effect will last for several. It peaks which predisposes people to low sugars. Newer insulins replace nph for better control.

what is the difference between novolin (insulin) n and novolog for a type two diabetic?

Onset/duration: Novolin n (a brand of nph) is an intermediate acting insulin. It starts working after a few hrs, peaks in 6-8 hrs and lasts 12-16 hrs. Novolog is a rapid acting insulin. It starts to work in a few mins, peaks in an hr or two, lasts 4 hrs or so. Novolog is used @meals to prevent post-meal high bs and to "correct" bs.  Nph (or better yet a long acting like Lantus (insulin glargine) or levemir) keeps fasting bs lower.
