my 18.month has a vocab of 10 words, otherwise still babbles but points & takes instruction like sit down, is that normal? how can i encourage talking

Yes: Just let it happen. Vocabulary increases as they pair up what they see with how the people in their environment use words. If you are doing something with them you can describe the setting and what you are using rather than be mute during the process.

my two year old son refuses to talk yet he understands everything we say and he follows instructions very well, but he just doesnt want to talk. he use his hands to show you want he wants. he only says daddy and mom when he needs my urgent help ?

is it healthy to use baby talk with my child or will it impede my childs language development?

Baby talk is fine.: Baby talk is a normal and good way to engage with an infant or young child who is not yet talking. At about 18 months when the child is imitating your speech and able to do so accurately, you want to make sure you are pronouncing words correctly but even then we adults speak very differently to young children with a sing song voice and a lot of repetition. This comes naturally to most adults.

i smell this odd smell. its hard to describe what it smells like. nobody else smells it.i feel embarrassed to talk to my dr. what might it be ?

Unpleasant odor:  Any dental problems?. If not, first consult an Otolaryngologist to check for nasal, paranasal sinuses or taste pathology. Other causes are  neurological abnormalities or psychiatric disorders(which is unlikely in your case).  If this phantom odor is only of recent origin, it may resolve.

This is very normal: At some point in his/her life, bascially everyone can say they have had a conservation with him/herself.  It only becomes a problem when it's intrusive in your life, as in prevents or distracts you from being able to being able to complete tasks that would otherwise do without issue.

Learn tricks: I'll let others address whether you have social phobia, autism-spectrum or whatever. You can talk to anyone using a few tricks. Repeat what they said. Talk about their interests. Find something you can truly admire about them &amp; let them notice. Relationships are more important than being right. Look / act interested. Avoid religion, politics, sex. The Dale Carnegie books are still great.
