Pain on passing POO: This sound more like endometriosis. Please see your GP for assessment as there may be other causes like anal fissure, haemorrhoids, constipation, Proctalgia fugax etc which need to be considered

: It is likely to be muscular deconditioning. You can address it with regular aerobic exercise. Any exercise will do as long as you it for about 30 minutes 5 days/week. Gradually increase the intensity so that you are sweating at the end. Raising core body temperature through exercise will improve strength and flexibility of your muscles.

Pooping and pain: When defecating or straining, you increase
your internal abdominal and pelvic pressure/force.  That force, is then transmitted to the tissues, muscles, ligaments and nerves in your back which the patient senses as back pain.

This typically signal that your lower back muscles, ligaments and joints are deconditioned and weak.  Therapeutic exercises to stretch, strengthen the abdominal and lumbar.

The act of 'bearing down' while defecating can increase the intradiscal pressure between spinal segments. Since the outer covering of the discs (annulus) contain nociceptive pain fibers, this can result in pain from a torn annulus when performing this so-called valsalva maneuver. This could also be some less serious issues but I would recommend you seek medical attention now to rule out serious issues. This is even more important if you are an older adult

When to go to ER?: It sure sounds like you should be in the emergency room today or at the very least at your family physician's clinic.  Gi bleeding can be life threatening. If you are having pain on top of that it sure makes me want to call 911 for you.

Probably Diet: If you have other symptoms like fever, sever local abdominal pain or  darkening urine, get medical evaluation. Have you tried non-prescription laxative, stool softeners (dss), glycerin suppositories? Consider adding a bulk former like psyllium seed (metamucil)  to soften your stool. Also, make sure you get at least 30gm of fiber daily.

Abdominal pain: Back pain may mimic  problems coming from the abdomen.  You should check in with your primary care physician  who can take a thorough history and physical examination to rule out an intra-abdominal source of your pain that may require very specific treatment.
