: If you did not lose consciousness and did not have changes in vision, hearing or thought process, you do not need to go to ER.
It appears that you posted this question a few days ago also and if you well enough to rewrite the question, you do not have brain damage.

Pain isn't concussio: Pain where you hit your head is normal. Concussion causes changes in vision, thoughts, alertness, or sensorium.
Can't say you don;t without exam, but doesn't sound like it

Doctor can evaluate: A knot or lump in the scalp or back of the head may be a lymph node or a skin cyst, among other possibilities. A primary care doctor can evaluate, and refer to an ENT or dermatology specialist if needed. It's very hard to guess exactly what it might be, without the whole story and an exam.

You are probably: straining the muscles in the back of your neck involuntarily. Ask a physical therapist to give you exercises to strengthen these muscles. It's not funny to be uncomfortable when laughing.

Your age is???: Your symptoms sound severe enough to warrent a visit to a doctor. Your age would suggest possible different problems in a female.  It could be things as diverse as pregnancy to diabetes to an infection.  The internet is not a place to make a diagnosis for this.  Take care of yourself, and see a physician who can do a proper history and physical and maybe run some tests.

Difficult case: whatever little information gleamed by the neurologist may not be enough to secure a definitive diagnosis- isomtimes the problem may need to "blossom" before a diagnosis can be made- for example, if the sensation happens on one side of the body, this symptoms preceeds by about a say, the painful rash seen with the shingles .

You need to be seen: Go to a doctor to have this looked at.  Must rule out vascular sources of headache.  Go to an er if this is new.

Headaches: Sorry to hear you are having a hard time.  Headaches can be very disruptive.   This sounds like something that you should see a doctor about.  Some problems in the upper spine can cause headaches.  Wondering if this is why it is worse when you lie down.  Workup for this will likely include some kind of imaging studies of your brain and your neck.  Again, contact your doctor to figure this out.
