Doctor can check: A pediatrician can check, or a pediatric dentist can check.  It sounds unusual to have an "orange" lump on the palate.  Was there any injury such as a milk bottle with milk that was too hot?  Details like that are helpful for the doctor.  Little white firm bumps on young babies' palates or gums are "epstein's pearls", but are not "orange".

Causes of tongue that looks patchy include: lichen planus (responds to steroids), white sponge nevus (familial, benign, permanent), smoker's leukoplakia (2% risk of cancer, needs biopsy), hairy leukoplakia (due to epsteinbarr virus), warts (due to papillomavirus), "thrush" (rx antifungals).

Maybe: Have it seen by the pediatrician and if the mom is eating a lot of carrots, maybe have her cut down if breast fed. If not consider a formula change.
