Not if baby is fine: We (doctors, moms, dads, neighbors, etc.) all want babies to sleep through the night by the time they are 3 months old. Parents with babies who sleep through the night are lucky parents. As long as a baby is growing fine on his measurements at the check-ups, and is healthy, parents don't need to wake up a sleeping baby at night.

Depends: Your child might had developmental incomplete closure of the spine wrapping sheets, called spinal dysraphism, or spina bifida, which might have some connection with outside. Or the spinal cord end (resembling a horse tail) is too tight sitting inside and would stretch the spinal cord further as child is growing, which called tethered spinal cord syndrome. MRI would help to diagnose.

Can determine: What sex the baby is @20 weeks gestation if they can see the genitalia. No ultrasound can determine the sex of a future pregnancy.

my wife is 3 month & 2 day preg. gyn did ultrasound & said probably baby boy but could also be the embirical cord so not sure. can he mix up ?

It seems : He is unsure because the umbilical cord is near the baby's groin and makes the determination of gender difficult. A followup ultrasound may help.
