Toe Nails: First you are not on any kind of dose for this problem that will be therapeutic.  There are several over the counter anti-fungals that can assist the oral medications to be more effective.  There is another approach with a prescription for the newer paint on medication known as penlac (ciclopirox).

The rapidity of clearance will obviously depend upon the severity of the condition, the organism causing it and its sensitivity to miconazole, your immune status and a variety of other variables.  Be patient.
