Toe Nails: First you are not on any kind of dose for this problem that will be therapeutic. There are several over the counter anti-fungals that can assist the oral medications to be more effective. There is another approach with a prescription for the newer paint on medication known as penlac (ciclopirox).

Onychomycosis: Nailbed infections start in the nail matrix and need to grow out from there. This process takes longer in the toenails than the fingernails (+/- 12 mo vs 6 mo). Treatment w/ fluconazole/terbinafin usually daily; some other agents (itraconazole) 1 wk/mo (or other permutations) w/75-80% cure. Sometimes the nail has to be removed. Newer topicals may as well help alone/in combo w/oral antifungals.
