Depends: You need to see orthopedic surgeon to see what your options are.  If surgery is indicated, consider stem cell therapy as well it might be a lifesaver and at your age i would avoid a hip replacement until you are much older.

Be happy!: At least a serious problem such as avn or significant osteoarthritis exists.  Your pain may be coming from trochanteric bursitis or your tensa fascia latus. See someone in orthopedics, physical medicine or rheumatology!

No: A torn rotator cuff primarily involves the rotator cuff tendons whereas avascular necrosis involves the humeral head (top of the long bone of the upper arm).

Each disease has certain features on mri.

Avascular can: may cause u problems down the road. Especially if you are an active person. You may want to get some functional orthotics to protect the navicular and repeat the x-rays every year.

Would not miss it!: Mri will pick up the earliest phases of avn, inclding subchondral marrow edema.  There is no more sensitive test.

Orthopedic Eval: I would strongly recommend that you go have an orthopedic evaluation,  preferably with an adult hip specialist.  Treatment for this can range from observation, medication, to surgery.   Treatment for avascular necrosis once the hip has collapsed requires a total hip replacement.   Sometimes surgeries can be done before the hip collapses to help preserve your native hip. Get it evaluated.

Can be both: Fractures of the navicular bone can be complicated by avascular necrosis if one fragment loses it`s blood supply.

Fall or trauma: Avascular necrosis is typically caused by a very hard traumatic blow to an area or repetitive trauma to an area that causes a lack of blood flow to the bone and therby creating the avascular necrosis.  You need to see your orthopedist.

Eventually, the joint may hurt even when you're lying down.Pain can be mild or severe and usually develops gradually.Pain associated with avascular necrosis of the hip may be focused in the groin, thigh or buttock.
