No.: Very unlikely that at age 17 this would be any type of cancer.  You may have severe premenstrual disorder, or possible an ovarian cyst causing the one-sided pain.
Please see a doctor to discern the causes of your pain, they can be treated so you aren't suffering.  You will also feel much relief to know that there is nothing scary going on.

Ilium or ileum?: This is a little confusing. The ilium is part of the bony structure of the pelvis. The ileum is part of the intestines. From the other part of your question I suspect you mean the ileum.  If so then this pain could be caused from constipation. It could however be something more serious. I would try to relieve the constipation and if the pain does not resolve then I would seek medical care ASAP.

SI joint strain: Judging from your question, it sounds like you might have sacro-ilium joint dysfunction. This can happen incidentally due to misalignment along the the lower part of the spine in relation to the plane of the hips. Therefore, one side can "pull" which causes the rotation. Usually physical therapy can help strengthen the weaker side and re-align the hips if someone is experiencing soreness and pain.

Yes, possibly: Your ilium is the large wing-shaped pelvic bone. The medial portion of your ilium is the part closer to your spine.  A lesion in your ilium could cause pain in your back and buttock area, but so could many other things  including a compressed nerve (from a bulging disc), a tight piriformis muscle,  or inflammation in your sacroiliac joint. It sounds like your pain "refers", making me think nerve.

Scoliosis or spasm: Either of these conditions may be the cause. A back muscle spasm or contraction may cause the pelvis to tilt, even briefly and be captured on the x-ray (radiograph).  Otherwise that finding , if described in a radiology report is likely not significant. Discuss with the doctor ordering the exam if you are still concerned.

Varies: There are multiple potential causes.  Certainly the concern for a neoplastic condition would be a consideration.  A serum protein electrophoresis can usually give a good idea about whether it is multiple myeloma.  A biopsy would certainly be  the definitive test.  Thank you for the question.
