No definite level: This is a complex question.  We would need to know what is the patient's problem .  If there is no immediate surgical issue, then transfusions are given if there are symptoms like chest pain.  If there is blood loss during surgery, then indications are different.  At our hospital, the medical staff has said there should be no transfusions unless the hemoglobin less than 8 gm/dl.

No: Why was blood transfusion given?  The reason for the initial drop in hemoglobin may still be present.

6-12 weeks: There are a number of variables here.  First, how low was your hemoglobin when you started?  Second, if you received a transfusion, what was your hemoglobin after?  It takes about 90 days to cycle through your blood supply, so after 3 months, you should be back to your baseline.

Hemoglobin level: Most centers will not transfuse a stable non bleeding patient with a hemoglobin over 7.  Speak with your doctor if you are having symptoms due to anemia. I know Acterma can cause decreased platelets and WBC but I did not know it caused anemia too.  Speak with your doctor.

Hemoglobin: A level of 4.9 is extremely low and would usually cause significant symptoms.  Hopefully the cause of your severe anemia has been addressed.

Wow: There must be an explanation for the anemia.  An effective treatment course can only be accomplished by having a proper diagnosis. Iron deficiency is the most common reason for a woman your age to be anemic, but there are many other diagnoses that must be looked for. Until you have that specific diagnosis you can't begin to prevent future problems.

See your doctor: You did not say why was your hemoglobin at 6.5. If the underlying cause was not corrected you could be headed for low hemoglobin again, as you surmised. It would be prudent to see your doctor at the earliest.

Critical problems!: Dialysis is needed urgently. A transfusion can wait, but depending on the circumstances, may be needed also.

Blood loss: When Hgb drops below 10 gm one has to be suspicious of blood loss but can be corrected by meds such as ProCrit (epoetin alfa).  When Hgb is in the 8 gm. range transfusion necessary to bring to over 12.  At 6.4 transfusion and work up for blood loss required.  Can be vaginal from uterine lesion, GI from ulcer or tumor but this must be defined and corrected.  Depending on symptoms appropriate physician needed.

Find the reason ....: Your doctor could investigate the underlying cause of your anemia... At age 37,  iron  deficiency is more common. Subsequently, appropriate  treatment could be offered. In case of  iron deficiency, iron tablets or intravenous iron could be prescribed. Hgb is not low enough to transfuse.  Would avoid transfusions if the underlying cause could be treated.
