No: Cracking your knuckles does not cause arthritis in the near term or long term.  This is a myth.  It's just annoying to some people, so i tell my teenage patients to avoid doing it around their parents (or other adults that it irritates).

See details: What did not rheumatologist say was the cause? Without pain it is unlikely to be arthritic inflammation. It is difficult to be specific with you without having the ability to examine the area first hand.

It's uncommon, but..: If you've had a significant injury including a fracture - especially one that extends into the joint, say of the thumb or the bones in the wrist - there might be the beginning of arthritis development.  An xray could help show if there's any arthritis there.  Best of luck!

Tylenol (acetaminophen): In a 75 year old it is important to know medical history, other meds, previous meds tried, kidney function and other variables before suggesting meds. Tylenol (acetaminophen) would be the safest bet.

Depends: This would depend on the kind of arthritis you have and what you mean about "hand therapy".  Many have plain and simple osteoarthritis of the hands and it is painful.  Warm parafin baths help.  Usually no "hand therapy" needed.  Some have rheumatoid arthritis and may need some splinting and other hand therapy and orthopedic treatment by a specialist in hand care.
