Block vs Re-entry?: The av node is the electrical connection between the upper chambers (atria) and the lower chambers (ventricles) 
there is av block (1st degree, 2nd degree - types i and ii, and 3rd degree)
1st degree is a just a slight delay in the time between the atria squeezing and the ventricle squeezing.
3rd degree is when the atria, that has the normal pacemaker doesn't send signals to the ventricles.

Pointed P waves: P wave is made up of the depolarization of the Right and Left atrium---on and EKG the first !/# of the wave is the RT atrium-----the last 1/3 is the LT atrium and the middle 1/3 is the summation of the RT + Lt  if the P wave is notched it usually is Lt atrial enlargement  if it is just peaked then it is usually Rt atrial enlargement---enlarging of the atrium especially the Lt can lead to a-fib

is a portable 1 lead ecg accurate to diagnose arrhythmias? or do i need a 12 lead?

Lead lead: If the printout or image of the recording in limb lead 1 is technically good without wandering baseline or other artefact and shows enough complexes(beats), it should be sufficient to diagnose many arrhythmias. Complex arrhythmias often are better identified when multiple leads are scrutinized, however.

is it possible to pick up serious conditions that might cause fatal arrhythmia (long qt etc) in a routine ecg/holter even if an episode is not caught?

It's possible: It is possible to pick up a long QT syndrome, without any episodes of passing out or rhythm problems.  Generally, you'd expect a QTc over 500 ms, before changes in treatment; however, you should see a heart rhythm specialist, or cardiologist, if QTc is unexpectedly prolonged.  Medications, gender, and heart rate can also affect your QT interval.  Holter measurements may be less reliable than ECG.

is it better to buy an at-home smartphone based 6-lead ecg (like alivecor kardiamobile 6l) for someone with intermittent chest pain and arrhythmia, because by the time a 12-lead ecg can be done in a clinic, the episode may already be over?

: Home EKG at the time of symptoms can be much better, depending on the quality of the home EKG, compared to the office EKG without symptoms. There are devices that record the EKG from days to months, that are attached to someone which is even better. In general, any testing, including EKG must be used in conjunction with seeing the entire clinical picture.

Yes but not serious: Palpitations can be part of normal extra beats for anyone. If they come and go lasting short time periods like less than a minute then usually, at your age, it's not serious. Concerning symptoms with palpitations include dizziness, fainting, severe shortness of breath or chest pain. To help reduce them, avoid excess of caffeine, alcohol, stimulants in cold medications, and reduce stress.

: Sinus rhythm with some variation (arrythmia) is a normal finding you want. The rest of the wording says the EKG is abnormal but is vague in why. This may just be a need for a better study. Please discuss this with the doc who knows your history &amp; can put it all into context.

hx of mild chest pains. normal ecg 2 yrs ago and no abnormal heart sounds in recent physical. should i worry about my concerta creating arrhythmias?

Concerta & heart: Since cardiac arrhythmias r possible serious reaction to methylphenidate & rapid heart beat & palpitations r common reactions, your cardiac function should be monitored closely for changes. I would also b very cautious about using caffeinated products (including NSAID combinations) & decongestants (the "d" in claritin-d, zyrtec-d, etc) as they may further stimulate rapid heart beat.

14 yo son had ecg which showed nsr with sinus arrhythmia and mild st elevation? could this ever be considered normal ? dr dont think needs a repeat ?

It IS: (in my opinion) NORMAL....ST elevation (minor) along with benign arrythmias are perfectly normal in kids!! Worse thing you can do is get him concerned about his "heart"!!

i suspect i have a arrhythmia disorder. however the ecgs are not catching these. because they can't catch them i can't get a monitor or cardiologist to track these. what can i do?

Holter: you should pursue a holter monitor or event recorder. I'm not your if your family doctor can order this where you live.
