VERY UNLIKELY: EVERY system has a safeguard consisting of a RECORDING of EVERYONE who accesses data.. 
This is a MAJOR DETERRENT to those who would "explore" data they are NOT entitled to!

Registries, not EMRs: Emrs include any patient records that are computerized - some track disease management and others don't. Registries (usually electronic today, but can still be done on paper) help a doctor keep track of all of their patients with a specific health conditions (diabetes, for example) and see which patients are due for what care, whose test results are at goal, and who may need a reminder to come in.

No way to tell: Depending on the type of EMR. Usually a new copy would be resent after each addendum.

Maybe, but..: Early stage, superficial, and pre-cancerous lesions may be amenable to endoscopic mucous resection. However, more extensive, or more deeply extending, masses will require  a wider resection to be sure it's removed more completely and safely. Getting a second opinion, or having another conversation with your care team, are good options.

It depends: Upon the cause. There are many possible mental and physical causes. Things that can help are relaxation, avoidance of toxins such as alcohol/tobacco/anabolics/opiates and certain medications, diet/exercise and weight loss, making sure hormonal levels are normal, etc. Check w/your doctor. At your age, stress and anxiety are most common.

Elevated ESR: An elevated ESR or erythrocyte (red blood cell) sedimentation rate is a non-specific laboratory test which may or may not be related to an underlying disease process. By itself it does not mean much and has to be interpreted according to the clinical context. Infection, inflammation, malignancy, pregnancy, rheumatic diseases, obesity, etc. Etc. Can cause an elevated esr.

ED in young man: It is not unheard of. But, the reason may be easier to treat. Do not be afraid to get a full evaluation from an expert in men's health.  Erections are more than 50% caused by one's brain or mental function and less than 50% about the body.

Once the worry is diminished many men find their erections have returned.
