Possible: 1 cm is not considered  very small  , current  practice of endoscopic screening , will be picked up much smaller than 1 cm , yes could be done with out sedation , but you will be very uncomfortable , and no reason to suffer , follow the advice of your doctor, a relatively low risk procedure

Too small to be bad: 4 mm is 1/6 th of an inch- very small.  Most doctors can tell if polyp looks benign- not cancerous or not.  But final results will come back if he took it out and sent it to the lab.  Also, such small polyps very rarely would be cancerous.

Polyp,fibroid: I feel there is enough concern to have a gynecologist evaluate and follow-up these uterine entities. Uterine polyps are usually benign, but transformation to malignancy can occur and for this reason a gynecologist's evaluation and follow-up is important.

Gallbladder Polyp: Most small polyps (less than 1 cm) are not malignant, however, when small polyps occur with other conditions (primary sclerosing cholangitis, etc) the risk may be increased. Larger polyps are more likely to develop adenocarcinoma (>1cm).

Should remove: You should get it it removed , 40 mm polyp is pretty big , due  villous component  will have higher incidence of cancer . Please get it removed if possible by endoscopy , speak to your doctor.

No danger: There is no danger. Polyps can cause snoring, multiple sinus infections, and promote allergy symptoms, and sometime depend on the size can cause difficult breathing. My advise see ENT for proper evaluation and treatment

Yes you should: Have the gallbladder removed. Small polyps (&lt;1 cm) can be watched. Anything bigger should be removed to be sure it is not a cancer.  However, most polyps, even 3cm ones , are simply benign growths.  If it is causing pain, then that is yet another reason to remove it.

If removed: Gall bladder polyp  will have  high incidence of malignancy especially if it is over 2cm, if gall bladder is removed no 'big deal' by a simple surgery, if it transforms to malignancy, cancer of gall bladder has  very poor prognosis.
