Isotretinoin: Better late than never. Cystic acne won't be cleared by antibiotics. It's not for the faint-hearted but it's probably  what you need. The internet is full of scare stories but the fast majority of folks with cystic acne are glad after they've done it.

When you press to expell the contents you likely tear some surrounding blood vessels.
