Pain with BM: Almost always in your age group this is from anal fissure NOT hemorrhoids
Taking a bulk agent daily with a softener taken with each meal it should improve
Topical nitroglycerin helps relax the muscle when you defecate and is needed in some cases to heal
Those that do not heal usually require sphincterotomy as outpatient with quick and excellent outcome

Hemorrhoid?: Hemorrhoid and anal fissure are common reasons for anorectal bleeding. Have your md evaluate it for proper diagnosis.

Hemorrhoid, fissure: you could have irritated a small blood vessel causing bleeding. Sometimes a small tear can also bleed. These tend to be painful, and hemorrhoids less so. Stool softeners can help prevent this. Heavy or persistent bleeding should be evaluated by your doctor.

Anal fissure: It would be unusual to cause rectal bleeding with a rectal thermometer. Be sure to use Vaseline on end of thermometer for lubrication when using it. If he has constipation or hard stools, he may have a tiny tear, like a paper cut size, that causes bleeding when touched or when passes bowel movement. Use Vaseline on rectum 2-3 times a day for few days, if no resolution, see MD.

Hemorrhoids: Are likely the cause here.Avoid straining and pushing during a bowel movem.Take a stool softener and drink a lot of fluids if you are constipated. Bleeding should never be taken lightly. I would recommend making an appt with your family doc and possible referral to a GI doc for a detailed exam and rule out other serious causes. If bleeding is heavy or if you feel light headed you need to go to er.

Treat the herpes: It's unusual to get anal bleeding from herpes, but if it's clear that's the cause, then treating the herpes (acyclovir, valacyclovir, famciclovir) is the solution.

Rectal Bleeding: The most common cause of this is hemorrhoid, which are dilated small veins that occur after recurrent pressure from hard bowel movements or childbirth, for example. Anal fissures - little cuts in the anus - can also happen from the same process. If it increases in frequency/ duration/severity then you may need to check it out. Likely not a big deal, though.
