FullRemoval Placenta: Since placenta is an interface of dense capillaries surrounded by high maternal blood flow, remnants of the placenta left within the uterus commonly result in maternal bleeding, sometimes massive, within the first few days after delivery; sometimes stopped/masked by uterine contraction shortly after delivery. Additionally, tears of the uterus or vaginal canal into rectal vault &gt; big problems, etc.

Allergic reaction: Amniotic fluid embolism is a rare condition.It's due to amnionic fluid entering the maternal circulation.This results in a combination of cardiogenic shock, respiratory failure and a coagulation disorder causing blood clots or bleeding.Although most cases occur during or shortly after delivery,on rare occasions it has occurred 48 hours after delivery due to a delayed allergic reaction to the fluid

Placental location: From what you are stating your placental location would not exclude you from a vaginal delivery. If your doctor has told you differently then there may be other reasons. Check with your Obgyn for further guidance.

Sonogram: Accessory lobes will always be connected in some capacity. They are usually seen by sonogram. Yes they can be missed during delivery. It would be hard to miss during a c section as the doctor is able to physically look inside of the uterus fairly easily.

how often does placenta accreta go undiagnosed even after a csection delivery?

Placenta accreta: A previously undiagnosed placenta accrete may be diagnosed at the time of a cesarean section.  This would occur when the OB is trying to remove the placenta but cannot because it is attached (accreta). On the other hand, if the placenta detaches easily, the uterine cavity is always explored so it would be rare to have an attached placenta left behind but a focal placenta accrete could be missed.

my wife is 29wk pregnant her recent doppler scan shows afi 6.3, placenta grade is 3 with small calcified area. are there chances of premature delivery?

No increased risk: the fluid index is a bit low but that is highly variable on how the sonographer displays the image.  The other ultrasound findings would not confer any increased risk of premature delivery in the absence other worrisome clinical findings like maternal hypertension eclampsia etc
