Ask for it.: An urine analysis is simple to do and can easily be done with your physical.  The prostate screening can also be easily done.  However, prostate screening has recently gone off the recommended list for standard screening by some organizations.  My advise would be to simply ask your physician to perform these test.

Bring all past info: Since it is a first visit he will take a detailed personal history, ask you about your family history of illnesses, your social history, do a thorough physical exam, a laboratory exam and then a discussion of the results with you. If you have any lab results from last year and if you are on any meds bring that info with you. And if you have particular questions write them out and bring them.

Breast masses: Ultrasound being negative does not completely diagnose all forms of breast masses.  Obtain a digital mammogram if the masses are still present to be more certain of their benign nature. Breast masses are common in a young woman, and are mostly benign at your age.  Get the mammogram which will be reassuring to you.

Age 40 but...: Thats if you have no strong family for heart disease , high blood pressure, diabetes, breast or colon cancer, non-smoker, normal bowel habits, no non-healing skin sores , maintain routine dental checkups, and have no long term / chronic complaints.
As always , this decision should be made between you and your family doctor.

Difficult: tough to answer in 400 letters or less.  Each individual has to look at the test for them.  If you are healthy, I would get the PSA test yearly along with exam.  It is a tool, not the Bible.  PSA can fluctuate but if you would see a rise in number too quickly, that would raise alarms to consider a biopsy.  In your case, I would check PSA yearly.

Doesn't matter: The gender of the physician and patient do not have to be matched to have a quality annual exam. If pub feel more comfortable with one over another, that's ok, and you can choose your doctor accordingly. Best to keep the same doctor over the years and develop a good relationship. It is important: It is important to tell you physician if you have a mental problem. You sound like you need to have  consuling and your physican can help you with your probems and have you see the correct physician If there is a need for you to be hospitalized your evaluation will tell that.
