when should i try to walk on my 3rd degree ankle sprain without crutches? is it normal to have a pain in all the ankle and toes (like nerve pain?)

Sprain: That is very dependant on the patient , but a general rule would be 3-4 weeks minimum.   If surgery was done even longer.  Ligaments can take as long as bones to heal and pushing walking too soon can result in re-injury with even worse consequences than a sprain.  Be patient - it takes time.  Good luck !

You should get an MRI of the lumbar region to asses the severity of the problem.

Pinched nerve: Yes, although more commonly the pain is in the back of the thigh or calf along the course of the sciatic nerve.

Yes: a pinched nerve in your leg can cause pain to radiate anywhere down the leg, depending on were the nerve is being pinched in your back, depending on the level.  Pain down to the top of foot is usually L5 nerve.

Swelling can cause: Nerve pain from pressure on the nerves. I don't think ankle spurs would cause pain by the calf, i thi ankle spurs would create pain at the site of spurs. Anytime, calf pain is present, the most important differential diagnosis to rule out is a clot.

Intense Foot Pain: When pain is intense at the foot level, you think about nerve pain or bone pain. If you have not done anything to possible bruise or break a bone, then the nerves from the neck, spine, and low back should be evaluated. Tibialis anterior: It sounds like possibly you have strained your tibialis anterior muscle and tendon. This can occurr in people who do a lot of treadmill running as the tibialis anterior has to rapidly dorsiflex the foot repeatedly. Usually a overuse type of strain. Rest it and ice the dosum of the foot where the tendon inserts.Make sure you are wearing a quality running shoe.

Sciatica: Small disc bulges can be asymptomatic or cause some of the symptoms you have described. Typically, the ankle reflex is more reflective of the L5/S1 disc level with S1 nerve involvement. However, it is possible the disc at L4/5 can affect the S1 nerve root if the disc has migrated lower in the spinal canal. A MRI scan done in the supine position may not show problems in other positons (sitting).

how can drinking milk for a lactose intolerant person cause severe muscle and nerve pain from the shoulder and back down to the leg into the ankle?

It shouldn't,: The type of pain that you are describing is not typical of problems due to lactose intolerance. The description that you give lends more toward an orthopedic, perhaps a back problem, or a neurologic cause. I would recommend a check up by your doctor or by an orthopedic surgeon or a neurosurgeon. Good luck.
