See if your doc can call the script to a chain pharmacy in your state that would then transfer it to a chain outlet in Tn. You should always plan to travel with a supply of meds that lasts.

Not usually: With most people the patella is a single bone of the knee. Some people may have a patella in two or more parts but they are usually fused togerther grossly..
A patella can have too much motion and be more prone to dislocation.

Usually trauma: Calluses form at sites of skin trauma and usually due to footwear if on the ankle. Changing shoes or adding a layer of protection/cushion at the site may help.  Frequent moisturizing may help too.

: You may watch and wait. If the patch more than doubles in size, bleeds, ulerates, other patches develop, you get fever or lose weight, see a doctor.
For healthy diet: http://www.choosemyplate.gov/., exercise 30 minutes/day, drink enough water daily so your urine is mostly colorless, have safe sex, no tobacco alcohol weed or street drugs. 
Get HPV vaccine.
