Sedation: No, sedatives need not be used whenever local anesthetics are administered. Anesthesia should be tailored to the surgery and patient's physiology. It isn't a cook book recipe.

General or MAC: If bone work is involved i would not consider local. You could have general or possibly local with sedation (mac). Talk to your surgeon.

No: That combination of surgery can be done together but not under local anesthesia. Ask your surgeon.

It should be: possible to safely obtain the surgery you request either with local, twilight or general anesthesia. If the anesthesiologist is concerned, a preoperative cardiac evaluation could be obtained easily and confirmation of your normal heart obtained.

No: If your surgery is two weeks away, any smoke you inhaled will be long gone. Try to stay away from smoke (tobacco or other) in general, but it won't be a problem in terms of anesthesia.

Anesthesia: You need to discuss this with your anesthesiologist.  It depends on the extensive of the surgery.

Maybe,maybe not: Since the procedure might break or knock out permanent teeth if you moved at all, i don't know if any anesthesia professional that would sign on for such a task.You just place your self at additional risk.If you don't trust the people doing the procedure find some new docs.

is a chin & jaw implant/augmentation with rhinoplasty risky? can i have them both done together under only local anestesia with sedation?

Mild risk.: Every surgery carries some risk. However, smaller, quicker procedures are generally less risky than complicated, longer procedures. A chin augmentation can be combined with a rhinoplasty and done safely under local anesthesia with sedation. I would suggest making sure your surgeon and/or anesthetist is acls certified, and that your surgeon is  certified by the american board of plastic surgery.

could medicare cover a rhinoplasty?

Unlikely: Medicare does not prequalify surgery, so most surgeons would require you pay ahead. Medicare would then only approve payment after if you had trauma that resulted in a nasal fracture that misshaped your nose and effected your breathing. Even then they would only pay to straighten it, not provide cosmetic changes.
